Patients
Your initial visit is very important. To make it the best it can be, we request that you take the time to acquaint us with you and your health by completing the information on our Patient Portal. When you set up your initial appointment, A New Way Forward will generate a user Id and password that will be emailed to you. You will need these credentials to access the Patient Portal.
The Patient Portal is where you can enter your information and in the future review visit notes, requests refills and appointments, and view your lab results.
Your initial visit will last approximately 75 minutes. We want to spend this time with you and that is why we feel it is so important to supply the information we discussed above prior to your appointment. We review this information before your scheduled visit in order to be prepared to be make the most of your visit and ultimately help you.
The cost for your initial visit is $400.00 and must be received in our office at least seven days prior to your appointment. If you have questions or concerns regarding this call us at 608-284-0202.
We request our established patients complete their inforamtion in the Patient Portal, providing all their background health information. The Patient Portal provides you access to great information and is another way to contact us …at your convenience.
Visit Pricing
30 minutes: $160.00
45 minutes: $240.00
60 minutes: $320.00
These visits will be billed as actual time spent with the provider.
We accept credit cards, checks and cash; however, please note we do not have change on hand.
Wellness Testing
Blood Tests / Lab Tests: If you pay for and receive a lab order or kit from A New Way Forward, it is important you have the lab work completed within sixty (60) days, as pricing structures and fees may change.
Genova Test Kits: Take home testing kits from Genova have revised pricing. If you have one of these kits, please contact A New Way Forward to review pricing revisions. Any pricing discrepancies resulting in increased costs will be invoiced to the patient.
